Pernah tak dengar terma 'karies'? Kalau pergi ke klinik gigi, karies merupakan salah satu sebab utama gigi sakit. Kalau kita kurang biasa dengar perkataan 'karies', mungkin kita lebih familiar dengan keadaan 'gigi berlubang'. 

Karies merupakan keadaan gigi yang sudah reput. Kalau korang pernah alami sakit gigi, kadang-kadang sampai tak boleh tidur, kemungkinan itu disebabkan oleh karies. Kalau sakit gigi ni, boleh buat kita tak ada mood. Dahlah sakit, rasa nak marah je 24/7 pulak tu.

Jadi, hari ni saya nak ajar apakah punca gigi kita berlubang, bagaimanakah gigi kita boleh berlubang, apakah faktor risiko gigi berlubang, cara untuk mencegah dan merawatnya.

PUNCA


Punca utama gigi berlubang ialah makanan manis. Walaupun ada banyak faktor lain yang main peranan, tanpa pengambilan gula, gigi kita tidak akan berlubang. 

Untuk gigi berlubang berlaku, 4 faktor perlu diambil kira; permukaan gigi, gula, bakteria dan masa. Saya akan terangkan serba sedikit untuk setiap faktor.


1. Permukaan gigi

Pada permukaan gigi kita, ada satu lapisan dipanggil biofilm yang terbentuk daripada air liur. Dalam lapisan ini terdapat protein yang membolehkan bakteria untuk melekat padanya. Bukan setakat protein saja, nutrien untuk bakteria juga tersedia membolehkan bakteria untuk kekal hidup. Biofilm ini akan sentiasa terbentuk pada permukaan gigi. Lepas gosok gigi sekali pun, lapisan ini akan terus terbentuk.


2. Gula 

Terdapat dua jenis gula yang available; intrinsic dan extrinsic sugar. Intrinsic sugar ni basically gula yang terkandung dalam membran sel makanan contohnya macam buah-buahan. Extrinsic sugar pula seperti gula-gula, cookies dan aiskrim. 

Gula yang menyebabkan gigi berlubang ialah extrinsic sugar. Oleh itu, sebagai langkah berjaga-jaga, kita kena kurangkan pengambilannya.
 

3. Bakteria

Tak semua bakteria akan 'makan' gula yang kita makan. Bakteria spesis seperti Streptococcus dan Lactobacillus dikenali sebagai cariogenic bacteria. Bakteria ini akan metabolise gula dan menghasilkan asid. Secara semula jadi, asid bersifat menghakis. Oleh sebab ini, asid akan menghakis enamel gigi sehingga berlubang. Apabila gigi sudah mula berlubang, enamel gigi tak akan dapat melekat semula menyebabkan gigi berlubang selamanya. Penyelesaiannya hanyalah dengan 'tampal' gigi. 


4. Masa 

Stephan Curve

Bakteria mengambil masa untuk menghakis gigi. Berdasarkan graf di atas, pH mulut kita yang secara neutralnya bersifat alkali akan turun ke tahap asid selepas makan. Apabila kita membiarkan pH mulut kita terus turun sehingga bawah pH 5.5 (pH kritikal), gigi kita akan mulut menghakis. Jadi, bagaimana nak naikkan semula ke pH alkali? Teruskan baca, nanti saya akan beritahu jawapannya!


FAKTOR RISIKO


Untuk faktor risiko, saya akan terangkan beberapa golongan yang lebih berisiko untuk mendapat karies.


1. Kanak-kanak

Rampant caries

Hal ini kerana struktur enamel gigi kanak-kanak nipis. Enamel merupakan lapisan putih pada crown gigi. Gigi berlubang adalah disebakan oleh enamel terhakis. Oleh itu, jika enamel kita nipis, maka cepatlah gigi akan berlubang kerana enamel yang ada sedikit sahaja sebagai perlindungan.

Selain daripada itu, kanak-kanak sering minum susu. Oleh itu, sangat penting untuk ibu bapa mendidik anak-anak untuk berhenti minum susu pada umur 2 tahun dan elakkan campur gula pada susu anak-anak. 

Karies kanak-kanak dipanggil 'rampant caries' disebabkan karies yang terdapat pada banyak gigi.


2. Perokok 

Merokok menyebabkan pengurangan aliran air liur di dalam mulut. Antara fungsi air liur adalah untuk membersihkan gula yang ada. Lebih kurang seperti menyapu sampah, penyapu itu air liur, manakala gula itu pula kekotoran atau habuk. 


3. Ubat yang menyebabkan mulut kering 


Antara ubat yang menyebabkan mulut kering ialah ubat untuk darah tinggi. Darah tinggi merupakan salah satu penyakit yang common di Malaysia. Jadi, jika anda ada mengambil atau mengenali sesiapa yang mengambil ubat untuk darah tinggi atau antihypertensive dan mempunyai simptom-simptom mulut kering, boleh rujuk pada dentist berhampiran. Sama seperti penjelasan pada point sebelumnya, kurang air liur bererti kurang gula yang dibersihkan daripada mulut kita. 


4. Warga emas

Apabila sudah menginjak usia, maka sel-sel akan mula mati. Begitu juga fungsi kelenjar air liur. Apabila kelenjar air liur kurang berfungsi, maka berkuranglah air liur yang terhasil. Nampak tak betapa pentingnya air liur dalam kesihatan mulut kita? Walaupun menggelikan, tapi tanpa air liur, siapalah kita!


5. Sjogren's syndrome

Sjogren's syndrome merupakan penyakit sistem immun. Antara simptom yang common ialah kekeringan mulut dan mata. Penyakit ini bersifat autoimmune di mana sistem immun kita secara tidak sengaja menyerang sel tisu, sedangkan antibodi sepatutnya menyerang partikel berbahaya. 


CARA PENCEGAHAN


Beralih pula pada point ketiga untuk post kali ini iaitu cara mencegah pembentukan karies atau gigi berlubang. Dalam Dentistry, saya diajar dengan konsep 'Minimal Intervention Dentistry'. Terma ini bermaksud, kita perlu mengekang treatment, sebaliknya perbanyakkan pencegahan. Sebagai pelajar tahun 3 Pergigian, untuk setiap pesakit yang saya rawat, saya diwajibkan untuk memberi Oral Health Education pada mereka. 


1. Kurangkan pengambilan gula

Terhadkan pengambilan gula pada waktu makan utama sahaja; sarapan, makan tengah hari, minum petang dan makan malam iaitu 4 kali sehari maksimum. 

Mengikut WHO, kuantiti gula yang disarankan untuk dewasa ialah 60g sehari bersamaan dengan 5 sudu teh. Jadi, cuba bayangkan berapa banyak gula yang kita ambil dalam sehari? Tak termasuk lagi gula yang kita tambah dalam masakan.

Tambahan pula, elakkan makan gula satu jam sebelum tidur. Jika anda masuk tidur pukul 12:00 malam, pukul 11:00 malam tak boleh makan gula lagi. Kalau tekak rasa haus, boleh gantikan dengan air kosong. 


2. Gosok gigi menggunakan ubat gigi mengandungi florida

Dalam Dentistry, saya diajar untuk mengesyorkan fluoride, fluoride dan fluoride. Florida dalam ubat gigi, florida dalam mouthwash, florida dalam air mineral, florida dalam garam, florida dalam susu hahaha. Semua sebab nak tekankan kepentingan florida punya pasal.

Jadi, sebelum beli ubat gigi, wajib periksa dulu bahan kandungan. Konsentrasi florida yang diperlukan untuk dewasa ialah 1500ppm (parts per million bersamaan dengan mg/L). Konsentrasi bawah 1000ppm adalah tidak efektif.

Florida penting untuk kuatkan enamel gigi. Seperti apa yang saya terangkan tentang Stephan curve, apabila pH mulut kita berada kurang daripada pH 5.5, gigi akan mula terhakis. Florida bertindak untuk menurunkan pH kritikal hingga ke bawah pH 5.5 supaya walaupun pH mulut berada pada pH 5.5, gigi masih tidak terhakis. 

Selain daripada itu, florida dicampur dengan phosphate dan kalsium dalam air liur, mampu membentuk struktur enamel yang terhakis selagi gigi belum berlubang. 

Ingat, kita perlu gosok gigi sekali sebelum tidur dan pastikan  jangan makan apa-apa selepas gosok gigi.


3. Floss sekurang-kurangnya sekali sehari

Kebiasaannya, karies akan mula terbentuk di celah gigi, tempat di mana susah berus gigi untuk capai. Kadang-kadang kita tak nampak yang sebenarnya karies dah mula terbentuk. Sebab itu pentingnya untuk kita floss. Jadi, celah gigi kita akan bersih daripada plak gigi.


4. Melawat klinik gigi 2 kali setahun

Buat masa sekarang, dengan COVID-19 yang tengah menular, lagi-lagi muncul pulak delta variant dan macam-macam lagi, untuk berjumpa doktor gigi kerajaan agak sukar. Kalau tak keberatan, bolehlah ke klinik swasta untuk buat check up.


CARA MERAWAT


Cara merawat karies bergantung pada sifat karies itu sendiri, sama ada aktif atau inaktif. Dentist akan periksa keadaan gigi, buat beberapa pemeriksaan seperti X-ray dan Pulp Sensibility Test untuk tengok keadaan gigi sama ada masih boleh dirawat atau perlu cabut atau dipanggil juga 'extraction.' Jadi, saya akan senaraikan beberapa pilihan rawatan yang mungkin akan dikemukakan. 


1. Tiada rawatan 

Tiada rawatan yang perlu dilakukan jika karies tidak aktif. Hanya perlu perbaiki cara penjagaan gigi dan penjagaan diet.


2. Tampal gigi

Ini adalah antara rawatan yang sering dilakukan di klinik. Dentist akan bersihkan bahagian gigi yang sudah reput, kemudian gigi akan ditampal ke bentuk asal. Antara bahan yang digunakan untuk tampal gigi ialah amalgam, composite dan glass ionomer cement. 


3. Root Canal Treatment (RCT)

Untuk rawatan ini, maksudnya akar gigi kita dah terdedah dan akar gigi dah dijangkiti oleh bakteria. Jadi, dentist akan bersihkan akar gigi kemudian akan tambah crown. 


4. Extraction 

Untuk gigi yang sudah tidak boleh diselamatkan kerana banyak struktur gigi sudah reput dan tidak mempunyai struktur yang cukup untuk tampal gigi. 


Itu saja untuk post kali ini, saya ada sertakan senarai sumber yang saya guna sebagai rujukan. Ada juga beberapa ilmu yang saya kongsi berdasarkan apa yang saya pelajari. Mungkin ilmu yang saya kongsikan tak seberapa, tapi harap dapat membantu serba-sedikit. Selamat menjaga kesihatan mulut!

Sometimes it's better to get it out. It festers and gets weird when you bottle it up. I don't know why, but it does.



Alex, Approximately is my second read by Jenn Bennett. My first read is Starry Eyes which I liked, and I like this one even better! I can see now why some people who read Alex, Approximately first will find Starry Eyes underwhelming.

Jenn Bennett is now officially my go-to contemporary romance author, alongside Emily Henry and Sarah Hogle! She writes books with a joyful ride and characters full of passion. Her main characters usually have a hobby in something which I found fascinating because I got to learn new things besides having fun reading. The depth of the characters' knowledge about their hobbies reflects how much Jenn Bennett's research has put and it's A LOT because she knows her shit. 


Alex, Approximately is about Bailey whom is a classic movies geek. She has been establishing an online friendship with a guy named Alex who lives in LA via a film app. The book starts off with Bailey touching down in LA from DC. She never informed Alex about the fact that she has started living with his dad which coincidentally lives in the same area as Alex's. Soon, at Bailey's summer job place, she met Porter Roth, an annoying, cocky and arrogant guy. But being a horny 17 year-old teenager that Bailey is, with all the hormones, of course Bailey falls for Porter. With Porter around, Bailey doesn't know where does Alex stand in her life.


This book will be SO MUCH BETTER if the synopsis doesn't give away the true identity of Alex. Honestly, don't read the synopsis before picking this book up. I never read the book but I watched people's reviews on Booktube, so I know who Alex is in really life. 

I was expecting a story about working in a cinema for some reason and I don't know why but my imagination was completely off. I thought what plot could be possibly done for film geeks but this book did so well to keep me intrigued all the time. At the end, I felt like trying out classic films too!


Porter and Bailey's chemistry is undeniable. I love seeing them together, growing emotionally and working the relationship healthily. Although I love their relationship, I must admit it is a bit too sudden and I couldn't clearly see why they would fall for each other earnestly. It seems more about lust than love. Despite that, I still enjoy reading about their dates together. It's not that cringe to me, but it might be for some people. 

Jenn Bennett without a doubt knows how to write male characters with great passion! Her male characters are normally so soft and gentle and looks at you when you're looking away and smile to themselves like they're thinking how grateful they are to be in your orbit. And UGH Porter is so cute to Bailey. 

And I think Bailey is such an adorable person. I was scared if she will be annoying as most contemporary characters would be (but not all) but I don't mind her. I don't think she's a snob or acts like not-other-girls just because she loves classic films and girls her age don't normally enjoy them. It's not like she shoves to everyone's faces about how much she loves classic films.


I never really realized how toxic and damning it would be to be an evader in life but seeing Bailey materializing in front of me is like seeing a part of me long buried deep. Evader is basically a person who avoids dealing with life problems as simple as avoiding to ask a person out in fear of getting rejected or avoiding that party invitation you have no idea how to turn down without sounding awful and anti-social or avoiding calls from bank demanding you to pay the debt you've been meaning to pay but never did. 

I resonate with Bailey being an evader at a personal level. Whenever I had an argument with my friends, I wouldn't dare to look them in the eye or even be in their sight because I don't want to face any unwanted conversation that might actually clear the argument between us but I just don't want to deal with it. And trust me, being an evader has caused me regrets after regrets in my life because I tend to avoid things until the very end, until it was too late for me to do anything. Hence, seeing Bailey grew up and worked her way out to be more honest and open with the people around her; her dad, boyfriend and bestfriend, made me want to pat her on the shoulder because I know how hard it is to face forward when evading is so much easier. But evading is never good both for you and the people around you. It could affect your emotional and ruin relationships. 

Avoidance is a dysfunctional way to interact with people you care about.

Despite my praise for Alex, Approximately, I have ONE thing that doesn't sit right with me like an itch on the sole of my foot. Porter's childhood best friend, Davy became a drug addict and did drugs like heroin after he was prescribed with opioids for his injury. I don't like how Davy was portrayed as the villain and yeah I know he did unforgivable things to earn money to buy drugs but his friends and the people around him clearly knows, it's even stated in the book about how Davy's family issues caused his addiction to spiral out of control. And NOBODY tried to help him at all. NOBODY tried to put him in rehab and CONTINUE badmouthed him. I think Davy's addiction is a very serious problem and it should be addressed properly. Davy deserved a better ending. 


Overall, I really enjoy reading Alex, Approximately and would recommend it to anyone who's looking for a summer-y read. Summer hasn't ended yet, so you still have time to pick this book up!

Doraemon: Stand By Me 2 is a sequel to Stand By Me 1 which was released in 2014. In my opinion, you can watch Stand By Me 2 without watching the first one. At least that was what I did. 



PLOT

In a nutshell, Stand By Me 2 is about Nobita who went back to his past to meet his late grandmother. His grandmother had one wish; to meet his future bride. And Nobita being Nobita, he promised her blatantly without thinking it through. When Nobita came back to the current world, he met his future self. During the time spent together, they swapped bodies using a special item. Little did they knew, the item was flawed and they met with a disastrous turnabout. 


The plot is simple, suitable to watch for kids and with family. I love the humour and there are scenes that made me actually laughed out loud. I had an enjoyable time with my siblings watching Stand By Me 2 together. Besides, the futuristic features in the adult Nobita life is fascinating. 

The movie is filled with warmth, joy and a great laugh. We got to see Nobita's parents younger selves and we got to see teenage Nobita bravely stood up to high school bullies when he switched bodies with adult Nobita. These are the scenes that cracked me up the most. 

I think Nobita's voice actor did a really great job. I'm a Malaysian and in Malaysia, we have Malay dubbed Doraemon. So, I have never heard Nobita speaks in Japanese. When I heard his voice, I think the voice suits him well. 




ART STYLE


I love the animation so much! The whole time I was watching, I gasped in awe. Look at the details in the trees? Can you imagine what the sky looks like? Spectacular. I particularly love the wedding hall, the interior design, decorations and cuisines are impeccable. 


SOUNDTRACK

I love the ending theme of this movie! I think it's such a karaoke song where you can sing along with your friends and be sad together. It sounds so nostalgic. Takes me back to when Doraemon in its early days.




CHARACTERS

All the characters are popular enough everyone knows what their personality is like. I still dislike Nobita because he's such a whiny brat 😂 But I love his friendship with Gian and Suneo in the future. 


OVERALL

I highly recommend to watch this movie with your family or friends! Something about laughing together and sharing jokes will make you feel warm and more enjoyable. This movie is in no way perfect, and it drags a little in the middle, but still worth a watch :)

Since it's lockdown and most of us can't go outside, we spend a whole lot of time at home and staring at the same home decoration for too long can only bore us. Recently, I just bought a proper study desk in my bedroom and I've been buying a lot of decoration items to liven up my study environment. So, here is a list of some of the items I actually bought and have been using for at least a week!


1. Digital Clock

This digital clock is aesthetic! I personally love the white colour. I put the clock on my nightstand, so it's easier for me to keep track with time as I'm reading in my bed. 

What I like most about this clock is it can change into different colours! 


2. Ceramic Mini Vase & Handmade Rattan Coaster

I bought the mini ceramic vase to put my new baby breath but it's too small 😅 The diameter of the vase opening is 1cm. So, I put a pen instead.

I love the contrast the rattan coaster gives against my white study desk. I think it looks beautiful. The rattan is sturdy. It can support a mug of drink. The coaster is available in many diameter, mine is 10cm. The measurement is correct without error. So, be sure to measure properly before placing an order! I think 8cm is quite small, so it's better to get the 10cm instead. 

I bought these two items during free shipping deals and even though it's from China, the order arrived at my house within a week. It was totally worth a buy!


3. Cute Mini Dustbin


I put this mini dustbin at the corner of my study desk and it's been so convenient for me! I can just throw away a piece of paper or tissue without having to walk over to a dustbin.


4. Baby Breath

I'm not a flower type of person but I'm so glad I bought this baby breath to give more colours in my study surroundings. The flowers are so cute and have been life changing.


5. Fairy Lights


This fairy light is available in 3 colours; white, warm and blue and the length are available in; 1m, 2m, 3m, 5m and 10m. I bought the 5m in warm colour. In my opinion, fairy light is a compulsory decoration in a bedroom because it makes the whole bedroom lights up and transforms the room to a princess chamber.


These are all my ideas for room decoration! I'm thinking about buying a proper vase for my baby breath and a scented candle. Would you recommend a nice and affordable scented candle? I'd love to know!
